The Department's Comments & Complaints Forms

Information you submit to us on our Comments Form will be transmitted through secure lines to our Office of Consumer Education and Advocacy. Information you submit to us on our Complaint Form will be transmitted through secure lines to our Office of Consumer Services.

Personal information from this web site will only be made available to others (e.g., employees or companies subject to PSC jurisdiction) as necessary to handle your comment or complaint, and will be used only for the purposes for which it was provided.

Access to and Correction of Personal Information Collected Through this Web site

Any user may submit a request to the Department privacy compliance officer to determine whether personal information pertaining to that user has been collected through this web site. Any such request shall be made in writing and must be accompanied by reasonable proof of identity of the user. Reasonable proof of identity may include verification of a signature, inclusion of an identifier generally known only to the user, or similar appropriate identification.
