What is a TCCI form?
By Secretary Notice dated April 21, 2005, issued in Case O4-C-1637, telecommunications companies under the Commission’s jurisdiction (telephone and cable companies) were notified of the requirement to submit a Telecommunications Company Critical Information (TCCI) form and that failure to file the form would result in a company being listed as “Inactive” on the Commission’s website. Since the April 2005 Notice, Department of Public Service (Department) Staff has sent yearly notifications to all regulated telecommunication companies requiring them to file TCCI forms annually. The information in the TCCI form assists in updating company information currently stored and maintained in the Department's Telecommunications Company Information Database that is maintained by the Office of Telecommunications. Any company experiencing a change in basic company information between annual TCCI filing cycles must submit a new TCCI form, at the time of the change, that updates the TCCI information accordingly (e.g., changes in company name, company address, contact information, etc).
Company information is utilized in a number of important Department applications including, but not limited to, emergency response situations when immediate communication with companies is needed. The annual submission of a TCCI form ensures that the list of regulated New York companies is accurate and contains up-to-date contact information. All contact information provided for each role on the form should be the direct contact information for the person holding that position.
Moreover, the TCCI form is used to facilitate companies' compliance with the Commission's Stray Voltage Order, issued July 3, 2006 in Case 04-M-0159, which requires all facilities-based telecommunications providers to attest annually to the Director of the Office of Telecommunications that they comply with the National Electric Safety Code and the National Electric Code.
The TCCI form is also the underlying data source of the Department’s public facing list of “Telecommunications Companies in New York State” found on the Department's website. The annual submission of a TCCI form ensures that the company contact information is accurate and up-to-date for Industry or consumer use.

What is an "Active" telecommunications company?
The term "Active" means that a company has completed a required TCCI form and has current contact information on file with the Commission.
What is an "Inactive" telecommunications company?
The term "Inactive" means that a company is not currently compliant with either the requirement to file an annual TCCI form or a TCCI form with updated information because a change to a company's name, address, or contact information has occurred. A TCCI form must be filed immediately whenever there is a change to the information contained in the form.
What is a "Ceased Operations" telecommunications company?
The term "Ceased Operations" means that a company is no longer providing regulated telecommunications service in New York State.
How can our company change its status from "Inactive" to "Active"?
If a company's name appears as "Inactive", it must file a TCCI form to have its name removed from the "Inactive" web page and placed on the "Active" web page of telecommunications companies. To submit a TCCI form, click here.
How can I update our company's information?
If there are any changes to information contained in a company's TCCI form, a complete, new TCCI form must be filed with revised information. Click here to view and submit a TCCI form. Upon Department Staff's review and approval of the form, a company's name will be removed from the "Inactive" web page and placed on the "Active" web page of telecommunications companies.
Our company is no longer doing business in New York State, what do we do?
If a company no longer provides regulated telephone or cable services, has no customers, and would like to surrender its Certificate of Public Convenience and Necessity (CPCN) or Certificate of Confirmation and relinquish authority to provide telecommunications services in New York State, the company should email a cover letter to the Secretary to the Commission at secretary@dps.ny.gov, stating the same and requesting to relinquish all rights associated therewith. For telephone corporations, the cover letter should be accompanied by a tariff Cancellation Supplement for each tariff in effect, canceling them in their entirety.
If the effective tariffs are filed in electronic format, include this completed Cancellation Supplement with the cover letter (include one supplement for each effective tariff on file) and file in the Electronic Tariff System (ETS).
If the effective tariffs are filed in paper format, include this completed Cancellation Supplement with the cover letter (include one supplement for each effective tariff on file) and email to the Secretary.
