Due to changes in the industry, the Office of Telecommunications is implementing a new form to be used to report CLEC service impairment problems, such as out-of-service conditions or other operations issues. EDR Guidelines apply, i.e. the reporting carrier need to exhaust all escalation process levels prior to filing a complaint with the Office of Telecommunications.
